Abstract: The service will aim to assist with the timely discharge of patients from acute hospitals to residential care homes. ​The trusted assessor will seek to ​:— reduce time from patient becoming medically fit for discharge to arriving at care homes; (Links to high impact change model for — managing transfers of care HICHM 1),— improve communication between hospital and care homes to aid timely discharge,— assist care home managers with assessment process (Links to HICM 3 and 6 multi disciplinary, multi agency discharge teams and trusted assessors),— provide liaison where there is a potential change of needs,— reduce the need for care home manager to visit hospital,— if possible leverage working together on this project to encourage wider working together between independent sector providers.
